Circumcision is a minor surgery in which a tight foreskin or prepuce skin is cut. It is done mostly in condition when prepuce skin is tight and unable to retract back in resting condition or in erect condition.
1 religious. Done in Muslim and Jews. Circumcision should not be done inpatient has hypospadias or epispadias
2 phomosis ~inability to retract back prepuce skin in flaccid or erect condition
3 paraphimosis is a condition where the foreskin can't be returned to its original position after being pulled back, causing the head of the penis to become swollen and painful.
4 recurrent infection (Balanitis) It is a condition where the foreskin and head of the penis become inflamed and infected.
Circumcision is daycare procedure and the patient is discharged in few hours. In adults, it is done in local anaesthesia and in children less than 8 to 10 years it is done in mild sedation plus local anaesthesia so they have to stay for 3 to 4 hrs after the procedure.
1 stapler / stitchless circumcision/ZSR circumcision: this is the best method and most commonly used by us. A stapler device will cut the skin and seal it with a silicone ring. No bleeding / least scar/ least pain / fast recovery
2 laser circumcision. Laser is used to cutting the skin so bleeding is less but sutures needed.
3 surgical circumcision - skin is cut by a knife. It is Traditional surgery, which is done by general surgeons, more bleeding / more pain more swelling / stitch scar and more aftercare needed but the cost is less

DORSAL SLIT / Preputioplasty – Here we don't remove the whole prepuce skin, but an incision is made on the anterior or dorsal part of the prepuce and the tight foreskin is loosened, the procedure is minor But – the COSMETIC look is INFERIOR TO CIRCUMCISION.
Frenuloplasty: it is done in a condition where prepuce skin can be retracted but there is a painful erection. The frenulum lie at the ventral aspect of the penis is tight in such men and just it’s cutting by frenuloplasty is needed
Normally prepuce skin does not retract in children up to 3 years so if the foreskin is unable to retract till age 4yr, then it needs a consultation with a doctor

A common procedure in which there is the surgical removal of the skin covering the tip of the penis is called Circumcision. It’s generally performed on a newborn boy often within the first 2 days of life and before he leaves the hospital. When a baby is 8 days old in the Jewish faith, it is performed in a special ceremony. Covering the head (glans) of the penis, boys are born with a hood of skin, called the foreskin. In order to expose the head of the penis, the foreskin is removed in circumcision. Stitches aren't needed in this procedure and it’s a quick procedure that causes very little bleeding. The procedure for older boys is a bit more involved as they can also be circumcised. Over the wound, a protective bandage may be placed after the circumcision and within a week to 10 days, it generally heals on its own. Many newborn males are circumcised in India due to religious reasons. Studies show that Circumcisionlowers certain health risks although it is not medically required. In fact, the benefits of the procedure outweigh the risks according to the American Academy of Paediatrics (AAP). Discuss the procedure for circumcision with the doctor who'll perform it once you decide your baby will be circumcised. Although paediatric surgeons, paediatricians, family practitioners, urologists and neonatologists can also perform circumcisions but many circumcisions on newborns are done by obstetricians. Within 1 to 2 days of birth, most healthy babies can be circumcised. However, with certain medical conditions circumcision is delayed for babies. Whether or not your newborn should wait to be circumcised will be decided by your child's examining paediatrician or neonatologist. For the procedure of circumcision, a baby is typically awake. Helping to hold him safely in place he is usually positioned in a moulded plastic seat. Before the procedure begins the penis and surrounding skin are cleansed with antiseptic.
During circumcisiona baby's pain can be lessened with the help of several safe and effective pain-control methods. Ahead of time, about the type of pain control your baby will have should be asked by your doctor. Where the incision will be made, in order to numb that area of the penis a local anaesthetic is used. Using a tiny needle,a numbing cream and/or medicine is injected in the penis in this procedure. As the needle goes through the skinyour baby will feel very little because of the numbing cream. To reduce new born distressa "sucrose pacifier" may be given to your baby by the nurse to further reduce stress and discomfort. Sometimes in order to help with painacetaminophen is given. When swaddled, babies may also feel better.
In order to keep the wound from sticking to the baby’s diaper his penis gets wrapped in gauze and doctors will apply petroleum ointment over the wound after a circumcision. Shortly after the procedure your baby will usually be brought to you. No matter which technique is used there is very little bleeding after circumcision. Though when you first take the dressing offyou may see a little bit of blood oozingon the diaper or from the edge of the incision which will generally stop on its own. Be gentle when bathing him because for a few days after the procedure your baby's penis may be sore. Special bath products and astringent should never be used. Keeping the area clean with warm water is recommended by most of the doctors. Soapy water can be used to gently wipe the poop away if it gets on the penis. For the first day or twowhenever you change a diaperapply a newdressing on the incision if there is one. For 3 to 5 dayson the front of the diaper or on the penis a dab of petroleum jelly should be put on by you even after the dressing is no longer needed. From rubbing and sticking to the diaper this can help avoid discomfort. For a circumcised penis to heal from the procedure it usually takes between 7 to 10 days. The tip might be raw or yellowish until it does. You must call your doctor right away if you experience any of the following problems:
A urinary tract infection is less likely to infect a circumcised infant in the first year of life. Although once the penis becomes retractableboys who don't have circumcisions can be taught to properly clean beneath the foreskin as it may be easier to keep a circumcised penis clean and uninfected. For developing cancer of the penis circumcised men may also be at lower risk according to the studies. From an infected female partner the risk of contracting HIV and other sexually transmitted diseases may get lower after having circumcision.
